You can upload your enrolment form below, along with the following information in order to make a successful submission:

Proof of identity, such as a copy of your passport, birth certificate or driving licence (a photo of the document can be uploaded as long as it is clear).
Proof of residency in the UK/EE, if you have not been in the UK/EE for the past 3 continuous years.
If you are on state benefits, a copy of your recent benefits will need to be uploaded.
If you are on low income (gross salary is less than £17,004), a recent wage slip will need to be uploaded.

Once your ID/evidence has been checked by our registry and compliance team it will be destroyed in a timely and secure manner.
